The patient portal is your secure online tool to access your health information, request refills or communicate with your care team. The portal can be used on computers, tablets and smartphones. 

 

Registered users: Your current login and password can be used to login. Once logged in, you will be asked to change your password and update your security questions and answers. If you have difficulty logging in or don't remember your password, contact us at (856)-546-3003 ext:3376.  First time user and do not have a username/password, please call the same number and we will be happy to get you setup.
